New Patients: Initial appointment, Please allow up to 2hrs due to health consultation, paperwork and then service/treatment following. If you are scheduled for an advanced health scan, allow up to 3 hrs, you will fill out paperwork when Dr. Hartman goes through her part(s) of the scan. If you are in a rush- PLEASE let Dr. Hartman know in advance. NOTE: All paperwork at this time is filled out in office. Many patients do not have printers, have forgot to bring their paperwork with them etc. Dr. Hartman is willing send you paperwork through email as long as you have a printer and don’t forget to bring it with you. However, due to HIPAA laws, to protect your PHI, pretty much all doctors offices have patients fill out in office. If you want Dr. Hartman to send your paperwork through snail mail, patients will be charged what it cost to send.
Lateness: You need to arrive 5-10 minutes before your scheduled time. If you are running late you need to call Dr. Hartman 765-860-2144. Depending on how late you’re running, Dr. Hartman reserves the right to deduct time from your scheduled appointment and/or cancel your appointment after 15-20mins w/a full charge of what you were scheduled. It is not fair to Dr. Hartman, and other patients if you are running behind and/or cannot make your scheduled appointment. Moreover, Dr. Hartman loses out when someone does not show up or cancels last minute/day of.
Returned Checks: There is a charge of $25.00 for all returned checks, plus the bank fee and amount of the service/treatment that was not covered due to the bouncing of the check. If your check bounces a second time, you will be required to pay $50 fee, bank fees, the amount that wasn’t paid and pay with cash from there after. There will not be a 3rd time. Cash will be required after the 2nd bounce check.
Delinquent payment: Will result in a charge of $20 per day starting with the day of your scheduledappointment and continuing each day until payment is received. As with any business payment is due at the time of your scheduled appointment -NO exceptions. You cannot go anywhere to render a service without payment!
Canceling & Cancellation Fee: Any missed appointment without a day before notice given in the time frame allowed with reminders, or 48hrs if booked online, will result in a minimum fee of $80 (60mins) and $100 (75mins) and/or half the price of the service/treatment you were scheduled depending on what service/treatment you were scheduled for, e.g. half prices: all scans, biofeedback sessions, immunology, integrative or naturopathic medicine, coaching, functional nutrition etc. It is appreciated and considerate to cancel ahead of time and would allow someone else the opportunity to get in. During specials a FULL price charge will be made for last minute/day of cancellations while the special is running -due to the low price drop & not being able to fill them with last min cancellations. This includes no call no shows. NOTE: Different treatments, services, and therapies all have cancelation fees depending on what they are and how long they take. If you’re scheduled for multiple services, you will get charged for each, due to the length of time scheduled for these multiple services. Refer to cancelation waiver(s) filled out, website, and contact page.
Scheduling and Rescheduling: Similar to Canceling, you’re welcome to text, call or email during business hours to schedule and reschedule any appointment. cancel in time given or rescheduling at least 2days before reminder is given. If you schedule online you will have a 48hr notice to cancel without being charged anything, however, rescheduling less than 48hrs will result in a charge.
